Required Before You Play

The liability insurance requires that we have a waiver signed by all participants. You cannot play without it being signed. Please complete the waiver before your first ice session — it only takes a minute and can be done online using the button below.

What's Included

The online waiver form includes a digital signature field (signed using your mouse or finger), a name entry field, a date field, and a parent/guardian signature section for participants under 18.
